Financial Information
Full payment for all routine office visits is expected at the time of service. If we are contracted with your health insurance, you will be required to pay a co-pay or deductible, if it has not been met.
Visa, MasterCard, Discover and American Express are acceptable forms of payment.
We also offer financing options for cosmetic procedures. Please ask a member of our staff for details.
We will be glad to file medical claims to your insurance company or Medicare for any hospital or major surgical care you may receive.
