On your first visit, the receptionist will provide an information sheet for you to complete. If you experience any difficulty with the forms, please don’t hesitate to ask for assistance. All of the information, including allergies, past medical and/or surgical history, etc., is strictly confidential and is only used for the delivery of quality health care.
Should Dr. Bailey have an emergency or urgent phone call, your understanding is appreciated. Staff members have been extensively trained to answer most questions. However, should the answer require Dr. Bailey’s attention, he will return the call as soon as possible.
If you are suffering from an emergency, please do not hesitate to visit the ER.
Prescription refills should be called in during routine working hours. Please have the pharmacy and prescription name, number and dosage.
